Golfer Registration Individual golfers and foursome registration need to complete the Golfer Registration Form. Golfing is only $125 per person which includes the following: Greens fees, your cart, complimentary driving range balls, dinner and registration for the Aviation Forum. We will also hand out box lunches for all golfers, a gift bag to include a nice golf shirt, hat, coffee mug, sleeve of golf balls, two complimentary drink tickets, plus much more! Event sponsors needing to register golfers under the allotment for you sponsorship should complete the Sponsor Golfer Registration Form.
Form Instructions: Please fill out the forms completely. This is to ensure a smooth and accurate registration process the day of the event. If you intend to register more than four golfers please print multiple copies of the form and submit all copies. FAX your registration form back to 480-333-3592. This is a private fax and all payment information will be protected. These forms are prepared in Adobe Acrobat. When you click on the link the document should open automatically. Please make your checks payable to "AZBAA" and mail, with registration form to the address listed below. To ensure you are registered promptly PLEASE fax your registration! If you experience any problems please contact us at dlaux@azbaa.org. IMPORTANT NOTICE: Submitting the registration forms does not automatically ensure your registration. Registration forms submitted will be followed by a confirmation from the AZBAA via email or phone. If you have submitted your registration form after all slots have been filled you will be notified immediately and any payments will be refunded in full. Registrations will be accepted on a "first come first serve" basis. Any remaining slots the day of the benefit will be made available for purchase. Registrants that do not cancel 24 hours or more prior to the event and do not attend will not be eligible for a refund. This is the only fair method, and since this is for charity we need to keep all slots available for those that are able to attend. If your schedule changes and you are unable to attend please contact us as soon as possible. We understand the nature of the aviation business and want to keep this fair to all that are interested in participating!
